The insurance specialists hold certificates as Certified Application Counselors (CACs) or Navigators. Our staff has assisted consumers explore their health coverage options for many years. What to Bring to Your Insurance Appointment
You will need to bring the following items for each member of your household to apply for coverage:
- Date of birth or Social Security numbers (if applicable)
- Green Cards/Naturalization Certificates
- Proof of the income your household receives (paystubs, self-employment ledger, social security statement, etc.)
- Tax return
- Employer coverage information (employer coverage tool form)
- Proof of medical insurance (if applicable)
- Healthcare.gov login information (if applicable)
- Proof of tribal enrollment (if applicable)
- Proof of life event within the last 60 days (loss of coverage, marriage, divorce, birth/adoption, became new legal guardian, change of immigration status, permanent relocation, death)

You can get health coverage for the rest of the year if you qualify for:
A Special Enrollment Period due to a life change
Certain life events, like losing health coverage (including Medicaid or Children’s Health Insurance Program (CHIP) coverage), moving, getting married, having a baby, or adopting a child, may qualify you to enroll in or change Marketplace health plans outside the yearly Open Enrollment Period (November 1 – January 15).
South Dakota Medicaid coverage has expanded. Adults ages 19 to 64, meeting the income limits and other general eligibility requirements (with or without private health insurance) may be eligible for FREE or low-cost South Dakota Medicaid including regular checkups, dental care, and vision care. Apply for Medicaid.
A Special Enrollment Period due to a recent loss of Medicaid or CHIP coverage
If you recently lost (or will soon lose) your Medicaid or CHIP coverage because your income is too high or other changes make you ineligible, you may qualify to enroll in a Marketplace plan through a Special Enrollment Period.
Medicaid, CHIP, or a new Special Enrollment Period based on income
Individuals, families, and children with certain incomes may qualify for Medicaid, CHIP, or health insurance through the Marketplace. If eligible based on estimated household income, you can apply any time.

Struggling to Pay?
Complete Health does not refuse service to anyone based on ability to pay.
All patients – insured and uninsured alike – are welcome to complete an application for our Sliding Fee Discount Program.
Patients who qualify are charged for services based on their income and family size. Patients should bring proof of income (for the last 30 days) to their first appointment. Acceptable proof of income includes:
- Pay stubs from the last 30 days
- Prior year tax return
- Supplemental Security Income (SSI)/Social Security Disability insurance (SSDI) benefit letter
- Child support
- Retirement benefits
- Proof of non-income stating the individual has no source of income at this time (letter from caseworker, letter from previous employer within the last 30 days, or tax transcript showing you have not filed taxes during the previous tax season)
Income is due within 10 business days and can be emailed to patientsupportstaff@chsd.care. By signing the sliding fee discount application you are confirming that all information shown is correct and you understand the requirements for proof of income.
